I happened to figure this out today: it's the late return fine charged by Ottawa Public Library, paid by me while renewing my library card.

It is a charge from the Ottawa Public Library. The charge for replacing a lost card is $5 (in 2020). It can also be a charge for depositing money on your card so that you can print, or for paying a late book fee.
